Reviving a Lapsed License
The following information will assist licensees in reviving lapsed licenses in NC:
21 NCAC 48G .0203 REVIVAL OF LAPSED LICENSE
- A license that has been lapsed less than one year may be revived by payment of the revival of lapsed license fee and the current year’s renewal fee and by completion of the revival form.
- A license that has lapsed more than one year but less than five years may be revived by completion of the revival form, and:
- completing 30 units (if reviving a physical therapist license) or 20 units (if reviving a physical therapist assistant license) of continuing competence as provided in the Rules in this Subchapter,
- payment of the revival of lapsed license fee, and
- payment of the current year’s renewal fee.
- A license that has lapsed more than five years may be revived by completion of the application forms; and
- passing the “PT exam” (if trained as a physical therapist) or the “PTA exam” (if trained as a physical therapist assistant);
- compiling at least 500 hours within the period of one year in the following manner: between 50 and 200 class hours of course work (ie, refresher course, continuing education, pertinent college courses) approved by the Board as designed to demonstrate proficiency in current physical therapy theory and practice, and the remaining hours working as an aide under the supervision of a licensed physical therapist; or
- endorsement of a current license in another state as provided by 21 NCAC 48B .0102.
To determine which method of revival is available, you must answer the following questions:
- If your license has been lapsed less than one year, go to 1 (below)
- If your license has been lapsed:
- more than one year and have a current, active license in another state, go to 2 (below)
- more than one year but less than five years and you do not have a current, active PT license in another state, go to 3 (below)
- over five years and you do not have a current, active PT license in another state, go to 4 & 5 (below)
Process for Reviving a Lapsed License
PLEASE NOTE: ALL FEES ARE NON-REFUNDABLE
Revival by Payment: (Only if license is lapsed less than one year):
- Complete the Revival Form. Click here for Revival Form
- Send $130.00 made by personal check, certified check or money order with the application form to the Board.
Revival by Endorsement:( license lapsed more than one year and current active license in another state):
- Complete the Revive by Endorsement Application form:

- Click here for the PTA form
- Send $150.00 by certified check or money order with the application form to the Board.
Two character references required from individuals who are not relatives and who have known the applicant for at least one year. These forms must be returned to the Board by the individual completing the reference.
Active license verification - Complete top section of Certification of Valid PT License in Another State and send to the state PT licensure board where the active, current PT license is held. This form must be completed and returned to NCBPTE by the other Board.

Revival with Continuing Competence: (license lapsed more than one year but less than five years and does not have a current, active license in another state or is choosing not to Revive by Endorsement):

- Send $130.00 by personal check, certified check or money order to the Board.
- Send completed documentation for Continuing Competence points (For appropriate documentation to send click here to see Rule .0109 for specific categories of activities)
- PT – 30 pts (must include 1 point of Jurisprudence Exercise, online at the Board website, https://www.ncptboard.org/OnlineServices/Secure/JurisprudenceMain.php
- PTA – 20 pts (must include 1 point of Jurisprudence Exercise (online at the Board website, https://www.ncptboard.org/OnlineServices/Secure/JurisprudenceMain.php
Revival by Examination: (license lapsed more than 5 years and does not have a current, active license in another state and is not choosing the Revive by Clinical work/500 hour option):
- Complete the Revival Form.
- Click here for the PT form
- Click here for the PTA form
- Send $150.00 to the Board by certified check or money order. There is a $370.00 exam fee to be paid directly to FSBPT.
- Two character references required from individuals who are not relatives and who have known the applicant for at least one year. These forms must be returned to the Board by the individual completing the reference.
- Send the application form and fee to the Board. Send the exam fee directly to FSBPT. (Questions? Go to www.fsbpt.org).
- Pass the National Physical Therapy Exam.
- Complete Jurisprudence Exercise I (online at the Board website) https://www.ncptboard.org/OnlineServices/Secure/JurisprudenceMain.php
Revival by Clinical work or 500 Hour option: (license lapsed more than 5 years and does not have a current, active license in another state and is not choosing the Revive by Examination option):
Note: Applicant must satisfactorily compile at least 500 hours within the period of one year in the following manner: Between 50 and 200 class hours of course work (ie, refresher courses, continuing education, pertinent college courses) that have been pre-approved by the Board and remaining hours working as an aide under the supervision of a licensed physical therapist, providing the Board authorized the training and the supervising physical therapist accounts for the clinical hours. When you receive your license:
- Know your reporting period.
- If you were licensed before January 1, 2009, you are in the reporting period which ends December 31, 2010.
- If you were first licensed in 2009, your reporting period began January 1, 2010 and ends December 31, 2011.
- If you revived your license in 2009 and:
- Your license had lapsed for less than one year, your reporting period is January 1, 2009 to December 31, 2010.
- Your licensed had lapsed for more than one year, your reporting period is January 1, 2010 to December 31, 2011.
- Know the Practice Act, Board Rules and Position Statements. They are available here.
- Know the Continuing Competence Rules. Familiarize yourself with the Board rules related to Continuing Competence. One point of Jurisprudence Exercise is MANDATORY; other points may be earned according to the rules from any category. Go to Continuing Competence Rules.
