Disclaimer for Disciplinary Action Summary

Position: Length of Time Disciplinary Actions Remain Posted on the Web Page - adopted March 09, 2000

When the Board takes disciplinary action against a licensee, a brief statement of the conduct and the sanctions imposed will appear on the web page for a period that coincides with the length of the disciplinary action, or one year, whichever is greater. Licensees who have failed to meet the conditions outlined in their Orders will remain on the web page for an extended period of time.

If a licensee has not fulfilled all of the requirements of the Order, then the name will remain on the web page until all requirements are met.

Chambers, Dania W., PTA (Warning)
Location:         Durham, NC, Durham County

Conduct:         Documenting and billing for treatment that was not provided.     
Discipline:       Warning with conditions and reimburse the Board for the cost of the investigation.  (Effective date– Oct 20, 2015)

Deuel, Leana O., PT (Warning)
Location: Raleigh, Wake County
License #: P10494
Conduct: Failure to complete documentation in a timely manner.
Discipline: Warning and reimburse the Board for the costs of the investigation. (Effective – February 1, 2017)

Enoh, Elizabeth E. (Warning)
Location: Raleigh, Wake County
License #: P8689
Conduct:  Documenting and billing for more time than spent with patient care
Discipline: Warning, reimburse the Board for the costs of the investigation and complete a course including documentation and billing.  (Effective – February 1, 2017)

Harvin, Dawn April, PTA (Suspension)
Location:         Asheville, NC, Buncombe County
License #:        A5360
Conduct:         Documenting and billing for two (2) treatments that were not performed.  
Discipline:       Suspension for 1 year, the imposition of which would be stayed so long as she complies with several conditions.  In addition, she must reimburse the Board for the cost of the investigation.  (Effective date– March 23, 2016)

Kidd, William Bryan, PT (Surrender of License)
Location:            High Point, NC, Guilford County
License #:            P11526
Conduct:              Stealing prescription medications from his patients for his personal use and abusing prescription medications
Discipline:           Surrender of license for a minimum of 1 year.  (Effective date – January 16, 2013)

Metzger, Raymond S., III (Suspension)
Location:        Pinehurst, NC, Lee County
License #:        P3854
Conduct:         Engaging in inappropriate conduct with a student while serving as her clinical instructor.
Discipline:       Suspension for 1 year, the imposition of which would be stayed so long as he complies with several conditions for a period of three years.  (Executed July 31, 2015)

Pashley, Emilie E. (Probation)
Location:         Charlotte, Mecklenburg County
License #:        P15695
Conduct:         Failure to complete documentation in a timely manner.
Discipline:       Probation for 1 year with conditions.    In addition, she must reimburse the Board for the cost of the investigation.  (Executed – June 29, 2016)

William, Angela Belinda, PTA (Warning)
Location:         Windsor, Bertie County
License #:        A4479
Conduct:        Abuse of a resident in a nursing home.
Discipline:      Warning and reimburse the Board for the cost of the investigation.  (Effective date– May 2, 2016)
