Disclaimer for Disciplinary Action Summary

Position: Length of Time Disciplinary Actions Remain Posted on the Web Page - adopted March 09, 2000; revised August 24, 2017

When the Board takes disciplinary action against a licensee, a brief statement of the conduct and the sanctions imposed will appear on the web page for a period that coincides with the length of the disciplinary action, or one year, whichever is greater. Additionally, licensees who fail to meet the conditions in their Orders will remain on the web page until all requirements are met.


Current Disciplinary Actions

Carter, Andrew K., PTA (Probation)
Location: Wilmington, NC, New Hanover County
License #: A5331
Conduct: Engaging in misrepresentation or deceit or exercising undue influence over a patient or former patient for the financial gain of the licensee.   
Discipline: Probation for 2 (two) years, prohibited from engaging in home health and other conditions.  Reimburse the Board for costs of the investigation. (Effective – January 10, 2020)

Harrington, Michael D., II, PT (Suspension)
Location: Charlotte, NC, Mecklenburg County
License #: P18147
Conduct: inappropriate sexual conduct with a patient; harassing, abusing or inappropriately touching a patient
Discipline: Consent Order – Suspension of license 18 months active, 2 years inactive with conditions and reimburse the Board for costs of the investigation. (Effective – April 9, 2020)

Hurley, Brandy Michelle, PTA (Surrender of License) 
Location: Wadesboro, NC, Anson County
License #: A2956
Conduct: Removing prescription medications from a patient’s home
Discipline: License surrender 6 months; Inactive suspension 18 months.  (Effective Date – December 14, 2017)

Jones, Melynda F., PTA (Revocation)
Location: Monroe, NC, Union County
License #: A668
Conduct: recording false or misleading data, measurements or notes regarding a patient; billing and charging for services or treatment not performed.
Discipline: Revocation and Reimburse the Board for costs of the investigation. (Effective – December 5, 2018)

Martin, Michelle L., PT (Suspension)
Location: Lexington, NC, Davidson County
License #: P13995
Conduct: Altering a license or renewal card by changing any information appearing thereon; using a license or renewal card that has been altered; and using a form of a license or a renewal card that was not issued by the Board or is not current.
Discipline: Suspension for 2 years, active for 3 months/inactive for 21 months with conditions and Reimburse the Board for costs of the investigation. (Effective – June 5, 2019)

Failure to respond to a Board notice in a timely manner and failure to complete a continuing competence audit.

Conduct: Recording false or misleading data, billing or charging for services or treatment not performed, failing to record patient data within a reasonable period of time ater interventions
Discipline: Warning; complete a documentation and billing course and reimburse the Board for costs of the investigation. (Effective – September 2, 2020)

Taylor, Clinton Ross, PTA (Surrender of License) 
Location: Hendersonville, NC, Henderson County

Conduct: Falsifying documentation, billing and charging for services not performed and charging fees not supported by documentation
Discipline: Surrender of license for a minimum of 3 years.  (Effective date – January 1, 2018)
