General Information

  • Expedited Licensure Process: All Military applications are processed in an expedited manner.
  • Definition: For the purposes of this document, The Board has defined Military as either:
    • Military Trained
    • Active Military Member
    • Active Military Member spouse
    • Veteran or
    • Veteran Spouse
  • Military Fee Waiver: All Military are eligible to have the application fee portion of payment waived.
  • Military Temporary Permits: Military applicants who have a verified license in another state may also be eligible for a temporary military permit to practice prior to completing the application for licensure.
  • Military Trained Applicants for PTA Licensure: Military Trained Applicants may be eligible to become licensed using military training as part of their educational requirement.
Apply for Licensure
The application is active for one year from the date of submission after which it will be destroyed if it has not been completed, including the furnishing of all documents and records that are requested. File to be destroyed per the state functional record retention schedule. It is the responsibility of the applicant to make sure all forms are received in the Board office. All applications expire after 1 year if incomplete.
